What You'll Do:
Proactive Patient Management: Manage a patient caseload with a focus on closing gaps in both clinical and non-clinical care, ensuring a resolution-driven approach.
Resource Coordination: Identify and connect patients with valuable community resources that can enhance their care and quality of life.
Patient Education and Empowerment: Provide clear and concise patient education on managing chronic conditions, promoting health literacy and self-management.
Medication Management: Oversee medication reconciliation, adherence, and refills, identifying and addressing potential medication concerns.
Service Coordination: Ensure timely delivery of essential services, including home health, durable medical equipment (DME), home infusion, and other critical needs.
Patient Advocacy: Address patient questions and concerns, fostering open communication and building strong relationships.
Referral and Appointment Management: Assist with or manage referrals and appointment scheduling to streamline patient care.
What You'll Bring:
Active Multi-State/Compact License (LPN/LVN) (NLC).
Proficiency in Microsoft Office Suite and strong technical aptitude.
Excellent written and verbal communication skills.
Ability to work independently in a remote environment.
High-speed Wi-Fi and a dedicated, HIPAA-compliant home office workspace.
Ability to sit for extended periods, speak clearly, listen attentively, and take detailed notes during telephone conversations.
Bonus Points:
Experience with Epic electronic health record (EHR) systems.
Bilingual proficiency.
Additional single-state LPN/LVN licensures.
