In this rewarding role, you'll play a vital part in coordinating and managing care for patients with chronic conditions, ensuring they receive the resources and support they need to live healthier lives.
What You'll Do:
Proactive Patient Management: Manage a patient census, utilizing a resolution-driven approach to address and close gaps in both clinical and non-clinical patient care.
Resource Coordination: Identify and connect patients with valuable community resources that can enhance their care and well-being.
Patient Education and Empowerment: Provide clear and effective patient education on chronic condition management, promoting health literacy and self-management skills.
Medication Management: Perform thorough medication management, including identifying potential concerns, reconciling medications, ensuring adherence, and coordinating refills.
Service Coordination: Assist in ensuring timely delivery of essential services to patients, such as home health, durable medical equipment (DME), home infusion, and other critical needs.
Patient Communication and Support: Address patient questions and create open lines of communication to understand their needs and provide empathetic support.
Referral and Appointment Management: Assist with or manage referrals and appointment scheduling to ensure seamless care coordination.
What You'll Bring:
Active Multi-State/Compact License (LPN) (NLC) or (LVN).
Strong technical aptitude, including proficiency in Microsoft Office Suite.
Excellent written and verbal communication skills.
Ability to work independently in a remote environment.
Ability to sit for long periods of time and effectively use a telephone.
Ability to process and document detailed information during phone conversations.
Bonus Points:
Experience with Epic electronic health record (EHR) system.
Bilingual proficiency.
Additional single-state LPN/LVN licensures.
Remote Work Requirements:
Reliable high-speed internet (Wi-Fi).
Dedicated home office or HIPAA-compliant workspace.
