What You'll Do:
Manage a patient census with a resolution-focused approach to closing gaps in both clinical and non-clinical care.
Identify and coordinate community resources that would benefit patients in managing their health.
Provide patient education to improve health literacy and support the management of chronic conditions.
Perform medication management, including reviewing potential concerns, reconciliation, adherence support, and coordinating refills.
Ensure timely delivery of services such as Home Health, DME, Home Infusion, and other critical patient needs.
Engage in open dialogue with patients to understand their needs, answer questions, and provide compassionate support.
Assist with referrals and appointment scheduling to ensure seamless patient care coordination.
What We're Looking For:
Active Multi-State/Compact License (LPN) (NLC) (LVN)
Strong technical skills - Familiarity with Microsoft Office Suite
Excellent communication skills - Both written and verbal
Bonus Points If You Have:
Experience using Epic EMR
Bilingual skills
Additional single-state LPN licenses
Remote Work Requirements:
Reliable high-speed Wi-Fi
A HIPAA-compliant home office or private workspace
Physical Requirements:
Ability to sit for extended periods while working at a computer
Strong verbal communication skills for telephone-based patient interactions
Ability to take detailed notes in real-time during patient calls
