Position Summary: :The Community Health Worker (CHW) works collaboratively with the care team to implement a person-centered plan of care and conduct regularly scheduled homes visits. Responsible for securing, developing and preserving trusting relationships within the community, the CHW will provide consumers and caregivers general support and encouragement. The CHW will achieve this by providing health management plans and goals and coaching consumers to effectively manage their chronic health conditions and self-care.
